Forms can be completed online by clicking on the links below. Each registered youth and adult is required to complete a Member Participant Health and Medical Form. Additionally, each Adult must complete the Troop 1161 Policies Acknowledgement Form.
All forms are due by the first regular troop meeting of the calendar year or if joining mid program year, by your second regular troop meeting.

Follow prompts for completion of forms.
a. Select "Continue in Browser" or download the app.
b. Enter your name and email and check the box to agree to use the platform.
c. Select the "Start" button to follow the prompts to complete the form.
d. Once all fields have been filled in, select the finish button and the form will be submitted.
Adult Member Participant Health and Medical Form - This medical form is required for all registered adults in Trail Life. This form does not include the medical exam that is required for high adventure activities.
Youth Member Participant Health and Medical Form - This medical form is required for all registered youth in Trail Life. This form does not include the medical exam that is required for high-adventure activities.
Troop 1161 Policies Acknowledgment Form - This policies acknowledgement form is required for all registered adults in Trail Life. This one form references all troop policies which are found on the Policies webpage.
Immunization Exemption Request - This form accompanies the Adult or Youth Weekend Health and Medical Record if exemption from vaccinations or immunizations is desired and checked on the form.
High Adventure Medical Form - This form accompanies the Youth and Adult Weekend Health and Medical Form. It is required for any event lasting longer than 72 hours or events that include high adventure activities. A doctor's exam and signature is required. It is valid for one year from the date of signature. This form MUST accompany the Weekend Health and Medical Form.
12-15 Passenger Van Use Waiver, Release, And Indemnity - The Trail Life USA (TLUSA) Health and Safety Guide specifies in Section VI under Transportation for Troop activities that, “If a vehicle is designed to carry 12 or more passengers, the driver must have a commercial driver’s license (CDL).”
Authorization Agreement for Voluntary Adult Caregiver - Chapter 34 of the Texas Family Code allows a parent to authorize certain relatives or voluntary caregivers in a Parental Child Safety Placement to take specified actions and obtain services on behalf of a child if the parent is unable to for some reason. As required by Senate Bill 1598, 81 Regular Session, DFPS developed The Authorization Agreement for Voluntary Adult Caregiver to assist families in meeting the requirements of the law. The form is not a DFPS-specific form and is intended for use by any family that wants to ensure a child's needs are being met in the parents' absence.
